Samuel H. “Buddy” Formby, 64, of Ellijay, Ga., passed away Monday, October 11, 2010 at Emory University Hospital in Atlanta, Ga.
A Memorial service will be held at a later date.
Byars Funeral Home, Cumming, Ga., is in charge of the arrangements.
